Special Announcement – Changes Related to Covid-19

Due to public health concerns related to Covid-19, we have made the following changes to the operation of the Income Tax Department:

  • Walk-in Income Tax services have been suspended
  • We are offering drop-off tax preparation (please click on the link below to see specific details)
  • The Village of Crooksville Income Tax filing and payment due dates have been extended to July 15, 2020

For your safety, please do not send your tax documents by email. Please drop off physical copies or send them by fax or regular mail.


The Income Tax Administrator is Lisa Osborn. Lisa has been with the Village of Crooksville since April 1995. She will be happy to help you with any questions you may have concerning the Crooksville Income Tax.

The Income Tax

The Village of Crooksville levies an annual tax of 1.5% on salaries, wages, commissions and other compensation as provided in the Village of Crooksville Ordinance 2015-2548. The municipal income tax provides funds for the general operating expenses of the Village, including the Income Tax Department, as well as for our Capital Improvements Fund.

Income Tax Return packets are available at the Crooksville Income Tax office in the Village Administration Building, located at 98 South Buckeye Street. The income tax forms are also available by clicking on the appropriate links in the sidebar on the right-hand side of this page.
